I certify that all information provided in this online credit application is true and complete and that I am at least 18 years of age. I authorize CreditIQ.com to rely on this information to make inquiries for the purpose of evaluating and verifying this online credit application, including obtaining credit reports and contacting my employer. I authorize CreditIQ.com, on my behalf, to forward my credit application, and the results of my application to nonaffiliated third parties that may assist me in obtaining financing or purchasing a vehicle, who may also obtain a credit report for this purpose. Such nonaffiliated third parties may include automobile dealers in my area, or third party marketers.

I agree that CreditIQ and any of it’s third parties, affiliates, agents, service providers or assignees may call me, leave me a voice message, a prerecorded message, an artificial voice message, or send me a text, e-mail, or other electronic message for any purpose related to the servicing and collection of my accounts with you, for surveys or research or for any other informational purpose related to this credit application, the servicing and collection of the resulting loan account (each a “Communication”). I agree that you and any of your affiliates, agents, service providers or assignees (and any of your assignee’s affiliates, agents or service providers) may call or text me at any telephone number associated with my accounts, including cellular telephone numbers, and may send an e-mail to any email address associated with my accounts. I also agree that you and any of your affiliates, agents, service providers or assignees (and any of your assignee’s affiliates, agents or service providers) may include my personal information in a Communication and may conduct a Communication using an automatic telephone dialing system. You will not charge me for a Communication, but my service provider may. In addition, I understand and agree that you and any of your affiliates, agents, service providers or assignees (and any of your assignee’s affiliates, agents or service providers) may always communicate with me in any manner permissible by law that does not require my prior consent.

I acknowledge that the information provided constitutes my credit inquiry or application to CreditIQ and to any arranger of credit or creditor to which my information may be disclosed to process my inquiry or application. I further expressly agree that any arranger of credit or creditor who receives my information may contact me at the calling numbers I have provided, including my cellular phone number, using a live caller, an automated dialer, or an artificial or prerecorded voice. I intend this consent to be effective even if my calling numbers are included in a do-not-call list or registry.

You agree that we (the lender, and the dealer) may obtain credit reports about you to process your application and for legitimate purposes related to any CreditIQ or third party lender auto loan you might receive (such as reviewing, collecting, and servicing the loan). Upon your request, we will inform you whether we requested credit report and provide the name and address of the consumer reporting agency that furnished the report. You also agree that we may verify your employment, income, and any other relevant information related to your application, and that we will retain the application whether or not credit is extended. You understand that a preapproval of an auto loan from any of our lenders may only be available for preapproved applicants who purchase automobiles from the inventory of participating dealers.

Under the Equal Credit Opportunity Act (ECOA), CreditIQ is prohibited from discriminating in any aspect of the credit transaction. Prohibited bases are: sex, marital status, race, color, religion, national origin, age (provided the applicant is of legal age), income derived from public assistance and the fact that the applicant has—in good faith—exercised any right under the Consumer Protection Act (i.e. the right to lodge a complaint). Alimony, child support, or separate maintenance income need not be revealed if you do not choose to have it considered as a basis for repaying this loan.

FLORIDA RESIDENTS: INSURANCE ANTI-COERCION STATEMENT FOR REAL OR PERSONAL PROPERTY

THE FOLLOWING STATEMENT IS REQUIRED UNDER RULE 69B-124.002 OF THE RULES AND REGULATIONS PROMULGATED BY THE CHIEF FINANCIAL OFFICER RELATIVE TO ANTI-COERCION AND RULE 69O-124.002 OF THE RULES AND REGULATIONS PROMULGATED BY THE FINANCIAL SERVICES COMMISSION RELATIVE TO ANTI-COERCION

The Insurance Laws of the State of Florida provide that the Lender may not require the Borrower to take insurance through any particular insurance agent or company to protect the mortgaged property.

The Borrower, subject to the rules adopted by the Chief Financial Officer or Financial Services Commission, has the right to have the insurance placed with an insurance agent or company of his choice, provided the company meets the requirements of the Lender. The Lender has the right to designate reasonable financial requirements as to the company and the adequacy of the coverage.

I have read the foregoing statement, or the rules of the Chief Financial Officer or Office of Financial Services relative thereto, and understand my rights and privileges and those of the Lender relative to the placing of such insurance.

MAINE RESIDENTS: You have free choice in the selection of the agent and insurer through or by which the insurance in connection with the requested loan is to be placed. Your decision to obtain any insurance product from an agent, broker or insurer of your choice will not affect the credit decision unless the insurance product you choose does not comply with the lender’s reasonable requirements.

NEW HAMPSHIRE RESIDENTS: If you are applying for a balloon payment contract, upon request and before entering into the balloon payment contract, you are entitled to receive a written estimate of the monthly payment amount for refinancing the balloon payment in accord with the creditor’s existing refinance programs. A balloon contract is an installment sale contract with a scheduled final payment that is at least twice the amount of one of the earlier scheduled equal periodic installment payments.

NEW YORK RESIDENTS: In connection with this application, we may request a consumer report on you. If you request, we will inform you whether or not a consumer report was requested and, if it was, of the name and address of the consumer reporting agency that furnished the report. Additional consumer reports may be ordered without further notice to you in connection with any update, renewal or extension of credit granted.

OHIO RESIDENTS: The Ohio laws against discrimination require that all creditors make credit equally available to all creditworthy customers, and that credit reporting agencies maintain separate credit histories on each individual upon request. The Ohio civil rights commission administers compliance with this law.

RHODE ISLAND RESIDENTS: Credit Reports may be obtained in connection with this application for credit.

VERMONT RESIDENTS: You authorize Dealer and any financial service provider with whom this application is shared, and each of their respective employees or agents, to obtain and verify information about you (including one or more credit reports, information about your employment and banking and credit relationships) that they may deem necessary or appropriate in evaluating your credit application. If your application is approved and credit is granted, you also authorize the parties granting credit and/or holding your account, and their respective employees and agents, to obtain additional credit reports and other information about you in connection with reviewing the account, increasing the available credit on the account (if applicable), taking collection on the account, or for any other legitimate purpose.

WISCONSIN RESIDENTS: No provision of a marital property agreement, a unilateral statement under Wis. Stat. 766.59, or a court decree under Wis. Stat. 766.70 applied to marital property adversely affects our interest unless you furnish a copy of such agreement, statement, or decree to us or we have actual knowledge of such adverse provision before credit is granted.

CONSENT FOR ELECTRONIC SIGNATURES, RECORDS, AND DISCLOSURES (“E-Consent”)

Please read this information carefully and print a copy and/or retain this information for future reference.

Introduction. You are submitting a request to be matched with one of our third party lenders.  In order to offer you a loan, the third party lenders need your consent to use and accept electronic signatures, records, and disclosures (“E-Consent”). This form notifies you of your rights when receiving electronic disclosures, notices, and information. By clicking on the link assenting to our terms, you acknowledge that you received this E-Consent and that you consent to conduct transactions using electronic signatures, electronic disclosures, electronic records, and electronic contract documents (“Disclosures”).

Option for Paper or Non-Electronic Records. You may request any Disclosures in paper copy by contacting the third party lender directly. The lenders will provide paper copies at no charge. The lenders will retain all Disclosures as applicable law requires.

Scope of Consent. This E-Consent applies to all interactions online concerning you and the third party lender and includes those interactions engaged in on any mobile device, including phones, smart-phones, and tablets. By exercising this E-Consent, the third party lender may process your information and interact during all online interactions with you electronically. The lender may also send you notices electronically related to its interactions and transactions.  Disclosures may be provided online at our or third party lenders’ websites, and may be provided by e-mail.

Consenting to Do Business Electronically. Before you decide to do business electronically with the third party lenders, you should consider whether you have the required hardware and software capabilities described below.

Hardware and Software Requirements. To access and retain the Disclosures electronically, you will need to use the following computer software and hardware: A PC or MAC compatible computer or other device capable of accessing the Internet, access to an e-mail account, and an Internet Browser software program that supports at least 128 bit encryption, such as Microsoft® Internet Explorer, Netscape® or Mozilla Firefox®. To read some documents, you may need a PDF file reader like Adobe® Acrobat Reader X ® or Foxit®. You will need a printer or a long-term storage device, such as your computer’s disk drive, to retain a copy of the Disclosures for future reference. You may send any questions regarding the hardware and software requirements directly to the third party lenders.

Withdrawing Consent. Your E-Consent for our matching service and for our third party lenders’ consideration of your matching request cannot be withdrawn because it is a one-time transaction.  If you are matched with one or more third party lenders, you are free to withdraw your E-Consent with those third party lenders at any time and at no charge. However, if you withdraw this E-Consent before receiving credit, you may be prevented from obtaining credit from the lender.  Contact the third party lender directly if you wish to withdraw this E-consent.  If you decide to withdraw this E-Consent, the legal effectiveness, validity, and enforceability of prior electronic Disclosures will not be affected.

Change to Your Contact Information. You should keep third party lenders informed of any change in your electronic address or mailing address. You may update such information by logging into the third party lender’s website or by sending the lender a written update by mail.
